Primary Consultation & Assessment

Every therapy journey at Thrive Well Wellness Care begins with a detailed home-based assessment. During the primary consultation, our Occupational Therapist evaluates the patient’s physical abilities, motor skills, sensory responses, cognitive function, and daily activity performance. Based on this assessment, a personalized therapy plan is created with clear goals and structured sessions.

  • Home & Environmental Assessment: Identification of environmental barriers and recommendations for safer, more accessible living spaces.
  • Assistive Device Recommendation: Guidance on adaptive equipment, splints, wheelchairs, walkers, and home modifications if required.
  • Personalized Therapy Plan: Individualized occupational therapy program designed according to the patient's goals and functional abilities.
  • Caregiver & Family Education: Practical training and guidance for caregivers to support rehabilitation and improve patient independence.
